Abstract
INDONESIA:
Shigellosis merupakan infeksi usus akut yang disebabkan oleh Shigella flexneri dan ditandai dengan proses inflamasi yang memicu peningkatan sitokin proinflamasi seperti TNF-α serta aktivasi mekanisme antiinflamasi melalui peningkatan IL-10. Meningkatnya resistensi antibiotik terhadap terapi lini pertama Shigellosis mendorong perlunya alternatif pengobatan berbasis bahan alam. Kayu manis (Cinnamomum verum) diketahui mengandung senyawa bioaktif, terutama flavonoid dan triterpenoid, yang berpotensi sebagai agen antibakteri dan antiinflamasi melalui penghambatan jalur NF-κB serta modulasi produksi sitokin. Penelitian ini bertujuan untuk mengetahui pengaruh pemberian ekstrak kayu manis terhadap ekspresi TNF-α pada jaringan ileocecal junction serta kadar IL-10 pada mencit (Mus musculus L.) yang diinfeksi Shigella flexneri. Desain penelitian menggunakan post-test only control group dengan mencit Balb/c yang dibagi menjadi enam kelompok: kontrol sehat, kontrol negatif, kontrol positif, dan tiga kelompok perlakuan dengan dosis ekstrak kayu manis 2 mg, 4 mg, dan 8 mg. Induksi Shigella flexneri dilakukan secara intraperitoneal dengan dosis 5×10⁸ CFU/ml, sedangkan terapi diberikan selama tiga hari. Ekspresi TNF-α dianalisis menggunakan metode imunohistokimia, dan kadar IL-10 diukur menggunakan ELISA. Hasil penelitian menunjukkan bahwa pemberian ekstrak kayu manis mampu menurunkan ekspresi TNF-α serta meningkatkan kadar IL-10 secara dosis-dependent. Dosis 8 mg memberikan efek paling signifikan dalam menekan inflamasi. Simpulan penelitian ini adalah bahwa ekstrak Cinnamomum verum berpotensi sebagai agen antiinflamasi dan dapat menjadi terapi alternatif pada kasus Shigellosis.

Shigellosis is an acute gastrointestinal infection induced by Shigella species, with Shigella flexneri being one of the most common pathogens that causes severe inflammatory reactions. This infection activates NF-κB signaling, resulting in increased pro-inflammatory cytokines like TNF-α but also prompting compensatory rises in anti-inflammatory cytokines such as IL-10. The increasing antibiotic resistance in Shigella infections has led to the investigation of natural treatment options. Cinnamon bark (Cinnamomum verum) has bioactive constituents such as flavonoids, tannins, terpenoids, and saponins, recognized for their antibacterial, antioxidant, and anti-inflammatory effects. This study aimed to assess the impact of cinnamon extract on TNF-α expression and IL-10 levels in mice infected with Shigella flexneri. A genuine experimental post-test-only control group design was executed utilizing Balb/c mice categorized into six groups: healthy control, negative control, positive control (ciprofloxacin), and treatment groups given cinnamon extract at dosages of 2 mg, 4 mg, and 8 mg. TNF-α expression in ileocecal tissue was evaluated by immunohistochemistry, whereas IL-10 serum levels were quantified using ELISA. The findings indicated that cinnamon extract decreased TNF-α expression and elevated IL-10 levels in a dose-dependent pattern, with the 8 mg dosage exhibiting the most significant anti-inflammatory impact. The results endorse the efficacy of Cinnamomum verum extract as a natural medicinal agent for regulating inflammation in shigellosis. Additional investigation is necessary to identify active chemicals, improve extraction techniques, and optimize in vivo uses
