Abstract
INDONESIA:
Demensia merupakan gangguan neurodegeneratif yang ditandai dengan penurunan fungsi kognitif, umum terjadi pada lansia, dan dapat memengaruhi pelaksanaan ibadah haji. Deteksi dini demensia penting dilakukan sebagai bagian dari penilaian istitha’ah kesehatan jemaah haji. Penelitian ini bertujuan mengetahui gambaran hasil skrining demensia menggunakan Mini-Cog serta faktor-faktor yang berhubungan dengan kejadiannya pada jemaah haji lanjut usia di Embarkasi Surabaya. Penelitian observasional analitik dengan desain cross-sectional ini melibatkan 104 jemaah haji lanjut usia tahun keberangkatan 1446 H/2025 M yang dipilih secara purposive sampling. Instrumen yang digunakan adalah Mini-Cog, terdiri atas tes mengingat tiga kata dan clock drawing test (CDT). Skor ≤2 menunjukkan indikasi demensia positif, sedangkan skor 3-5 menunjukkan fungsi kognitif normal. Analisis data dilakukan secara deskriptif dan bivariat menggunakan uji chi-square. Hasil menunjukkan 26,9% responden (28 orang) terindikasi mengalami demensia. Responden dengan hasil skrining positif umumnya adalah lansia muda, perempuan, berpendidikan rendah, bekerja sebagai wiraswasta, tidak memiliki penyakit penyerta mayor, dan tidak sedang mengonsumsi obat. Faktor yang berhubungan signifikan dengan demensia adalah riwayat hipertensi (p = 0,047; OR = 0,321), penggunaan alat bantu jalan (p = 0,008; OR = 8,043), dan status minum obat (p = 0,006; OR = 0,270). Skrining Mini-Cog direkomendasikan dilakukan secara rutin, baik pada pemeriksaan istitha’ah awal maupun menjelang keberangkatan, guna mendeteksi dini penurunan fungsi kognitif.

Dementia is a neurodegenerative disorder characterized by a decline in cognitive function, commonly occurring in older adults, and may interfere with the performance of Hajj rituals. Early detection of dementia is essential as part of the health istitha’ah assessment for Hajj pilgrims. This study aimed to describe the results of dementia screening using the Mini-Cog and to identify factors associated with its occurrence among elderly Hajj pilgrims at the Surabaya Embarkation. This analytical observational study with a cross-sectional design involved 104 elderly pilgrims scheduled for the 1446 H/2025 M departure, selected through purposive sampling. The Mini-Cog instrument consisted of a three-word recall test and a clock-drawing test (CDT). Scores ≤ 2 indicated a positive dementia screening result, while scores 3–5 reflected normal cognitive function. Data were analyzed descriptively and bivariately using the chi-square test. The results showed that 26.9% (28 of 104) participants were screened positive for dementia. Those with positive results were predominantly younger elderly, female, low-educated, self-employed, had no major comorbidities, and were not taking any medication. Factors significantly associated with dementia included a history of hypertension (p = 0.047; OR = 0.321), use of walking aids (p = 0.008; OR = 8.043), and medication status (p = 0.006; OR = 0.270). Regular Mini-Cog screening is recommended, not only at the initial istitha’ah examination but also during the waiting period and prior to departure, to facilitate early detection of new cognitive decline.
