Identifikasi dan klasifikasi kejadian drug related problems pada pasien lansia diabetes melitus tipe 2 dengan hipertensi di Puskesmas Batu

World Health Organisation (WHO) mengungkapkan bahwa secara global terdapat 422 juta orang yang menderita DM di seluruh dunia. Penggunaan obat baik kombinasi dan tunggal dapat menyebabkan Drug Related Problems (DRPs). Penelitian ini bertujuan untuk mengklasifikasi DRPs pada pasien lanjut usia dengan penyakit Diabetes Melitus tipe 2 dengan hipertensi yang menerima terapi di Puskesmas Batu. Jenis penelitian ini observasional deskriptif yang dilakukan pada pasien Diabetes Mellitus tipe 2 di Puskesmas Batu. Identifikasi dan klasifikasi DRPs sesuai dengan Pharmaceutical Care Network Europe (PCNE) V9.1. Hasil Permasalahan terkait obat yang paling banyak ditemukan adalah reaksi obat merugikan yang telah atau mungkin terjadi pada 17 pasien (100%) dari domain efektivitas, diikuti oleh Masalah atau keluhan yang tidak jelas. Diperlukan klarifikasi lebih lanjut terjadi pada 17 pasien (100%) dari domain keamanan pengobatan. Penyebab Permasalahan terkait obat tertinggi adalah Tidak terdapat hasil pemantauan terapi obat yang sesuai yang terjadi pada 17 pasien (100%) dari domain terkait transfer pasien, diikuti oleh pasien tidak menggunakan obat sesuai anjuran pada 8 Pasien (47%) dari domain terkait pasien. Intervensi yang telah dilakukan apoteker kepada dokter berupa intervensi didiskusikan dan diusulkan kepada penulis resep, intervensi yang diberikan pada pasien berupa pemberian konseling. Penerimaan intervensi yang telah diberikan oleh apoteker dapat diterima tetapi tidak diketahui implementasinya merupakan yang tertinggi, yaitu sebesar 58,8%. Status DRP tertinggi adalah Tidak perlu atau tidak memungkinkan untuk menyelesaikan masalah 82,3%. Dapat disimpulkan bahwa DRP yang terjadi yaitu potensial masalah terjadi pada 17 pasien.

The World Health Organization (WHO) reports that approximately 422 million individuals worldwide are affected by diabetes mellitus (DM). The use of pharmacotherapy, either in combination or as monotherapy, has the potential to result in Drug-Related Problems (DRPs). This study aims to classify DRPs among elderly patients diagnosed with type 2 diabetes mellitus and hypertension receiving treatment at Puskesmas Batu. This descriptive observational study was conducted on type 2 diabetes mellitus patients at Puskesmas Batu, with DRP identification and classification based on the Pharmaceutical Care Network Europe (PCNE) version 9.1. The most frequently observed DRP was adverse drug reactions that had occurred or were likely to occur, identified in all 17 patients (100%) under the domain of treatment effectiveness, followed by unclear complaints or issues necessitating further clarification within the domain of treatment safety, also found in 17 patients (100%). The most prevalent cause of DRPs was the lack of appropriate therapeutic drug monitoring, observed in all 17 patients (100%) under the domain related to patient transfer, followed by non-adherence to prescribed medications, which occurred in 8 patients (47%) within the patient-related domain. Pharmacist interventions toward physicians included discussions and recommendations to prescribers, while interventions provided to patients included counseling. The most frequent outcome of pharmacist interventions was “accepted but implementation unknown,” accounting for 58.8%. The most common DRP status was “no need or not possible to solve the problem,” comprising 82.3% of cases. It can be concluded that the DRPs identified represent potential issues occurring in all 17 patients.
