Analisis efektivitas biaya terapi kombinasi insulin dengan obat antidiabetes oral pada pasien rawat jalan penderita diabetes melitus tipe 2 di RSUD dr. Soehadi Prijonegoro Sragen

Diabetes melitus tipe 2 (DM 2) adalah penyakit kronis menahun karena resistensi insulin atau akibat dari berkurangnya sekresi insulin oleh sel beta pankreas. Pengobatan DM 2 berupa terapi oral dan insulin. Cost-Effectiveness Analysis adalah metode analisis untuk mengambil keputusan alternatif terbaik dari terapi yang digunakan. Tujuan penelitian ini mengetahui efektivitas biaya terapi kombinasi insulin dengan antidiabetes oral (ADO) pada pasien rawat jalan di RSUD dr. Soehadi Prijonegoro Sragen tahun 2021.

Metode penelitian ini non-eksperimental deskriptif dengan pengambilan data secara retrospektif. Data yang diambil yaitu data rekam medis pasien rawat jalan DM 2 di RSUD dr. Soehadi Prijonegoro Sragen tahun 2021. Parameter yang digunakan yaitu kadar gula darah puasa (GDP) dan biaya medis langsung meliputi biaya pendaftaran, biaya jasa dokter, biaya obat dan biaya laboratorium.

Pasien yang memenuhi kriteria inklusi sebanyak 49 pasien. Terdapat 5 macam kombinasi obat antidiabetes yang diberikan kepada pasien yaitu insulin + ADO (61,22%), insulin + 2 ADO (8,16%), 2 insulin + ADO (14,28%), 2 insulin + 2 ADO (4,08%), dan insulin + 3 ADO (4,08%). Efektivitas terapi dilihat dari jumlah GDP normal dibagi jumlah total pemeriksaan GDP dikali 100%. Efektivitas terapi tertinggi 100% dan terendah 0%. Biaya medis langsung tertinggi Rp. 1.066.520 dengan terapi Apidra + Sansulin + Metformin + Pioglitazone. Terapi antidiabetes yang paling cost-effectiveness yaitu terapi dengan ACER terendah dan ICER negatif yaitu terapi Sansulin + Metformin dengan ACER sebesar Rp. 4.320 dan ICER sebesar Rp. -13,381.

Type 2 diabetes mellitus (DM 2) is a chronic disease due to insulin resistance or reduced insulin secretion by pancreatic beta cells. Treatment of DM 2 in the form of oral therapy and insulin. Cost-Effectiveness Analysis is an analytical method to decide the best alternative from the therapy used. This study aimed to determine the cost-effectiveness of insulin combination therapy with oral antidiabetic (ADO) in outpatients at dr. Soehadi Prijonegoro Sragen in 2021.

This research method is descriptive and non-experimental with retrospective data collection. The data taken are medical records of DM 2 outpatients in RSUD dr. Soehadi Prijonegoro Sragen in 2021. The parameters used are fasting blood sugar (GDP) levels and direct medical costs, including registration fees, doctor fees, drug costs and laboratory fees.

Patients who met the inclusion criteria were 49 patients. There are 5 types of antidiabetic drug combinations given to patients, namely insulin + ADO (61,22%), insulin + 2 ADO (8,16%), 2 insulin + ADO (14,28%), 2 insulin + 2 ADO (4,08%), and insulin + 3 ADO (4,08%). The effectiveness of therapy is seen from the number of normal GDP divided by the total number of GDP examinations multiplied by 100%. The highest therapeutic effectiveness is 100%, and the lowest is 0%. The highest direct medical costs Rp. 1,066,520 with Apidra + Sansulin + Metformin + Pioglitazone therapy. The most cost-effectiveness of antidiabetic therapy is therapy with the lowest ACER and negative ICER, namely Sansulin + Metformin therapy with ACER of Rp. 4.320 and ICER Rp. -13,381.
