Abstract
INDONESIA:
Gagal jantung merupakan kondisi abnormal jantung yang kompleks sehingga penderita gagal jantung cenderung menerima terapi lebih dari 5 obat. Banyaknya obat yang dikonsumsi pasien akan meningkatkan probabilitas terjadinya interaksi obat. RSUD Jombang terletak di Jawa Timur, provinsi dengan penderita penyakit gagal jantung terbanyak, dan belum pernah menjadi lokasi penelitian mengenai interaksi obat. Sehingga, dilakukan penelitian deskriptif observasional secara retrospektif untuk mengetahui gambaran pola penggunaan obat dan potensi interaksi obat pada terapi pasien gagal jantung rawat inap di RSUD Jombang. Sebanyak 72 sampel dipilih dari total 276 pasien gagal jantung sepanjang Januari-Desember tahun 2016 menggunakan systematic random sampling.
Golongan diuretik, golongan nitrat, aspirin, clopidogrel, dan ACE-Inhibitor menjadi obat yang paling banyak diberikan pada pasien. Pasien menerima 3 hingga 16 meliputi obat oral, intravena, dan subkutan (15,28%). Pasien dominan (31,95%) mengkonsumsi 5-6 obat. Potensi interaksi obat ditemukan pada 79,17% pasien dengan total 136 kasus yang terbagi menjadi 32 pasangan interaksi obat. Potensi interaksi obat paling banyak ditemukan adalah pasangan furosemide dan aspirin dengan efek penurunan vasodilator. Efek-efek interaksi obat yang berbahaya bagi pasien gagal jantung yakni hiperkalemia oleh spironolakton dan lisinopril, spironolakton dan KCl, serta lisinopril dan KCl; peningkatan resiko bleeding oleh obat-obat hemodinamik; dan peningkatan toksisitas digoksin oleh obat digoksin dan simvastatin.

Heart failure is a complex abnormal condition of heart failure. Therefore patients suffering heart failure usually receive more than 5 drugs. The amount of consumed drug will increase the probability of drug interaction. RSUD Jombang is located in East Java-the province with the most existed heart failure patients never be object of any researches about drug interaction. Thus a descriptive observational research was retrospectively held to know drug use pattern and potential drug interaction in the therapy of hospitalized heart failure patiens in RSUD Jombang. There were 72 sample chosen among 276 total patients of heart failure during January to Desember 2016 using systematic random sampling.
Diuretics, nitrates, aspirin, clopidogrel, and ACE-Inhibitor were the most used drugs. Patients received 3 to 16 drugs that were oral, intravenous, and subcutaneous drugs (15,28%). Most patients (31,95%) consumed 5 to 6 drugs. Potential drug interactions were found in 79,19% patients. There were 136 cases divided into 32 drug pairs. The most common potential drug interaction was furosemide and aspirin of which the effect is decreasing vasodilatory effect. Effects needing more attention in heart failure patients were hyperkalemia by spironolactone and lisinopril, spironolactone and potassium chloride, and lisinopril and potassium chloride; increasing bleeding risk by haemodynamic drugs; and increasing digoxin toxicity by digoxin and simvastatin.
