Efektivitas nanospanlastik krim ekstrak etanol kulit batang Parameria laevigata (Juss.) Moldenke terhadap penyembuhan ulkus diabetikum

Diabetes melitus merupakan penyakit metabolik kronis dengan prevalensi yang terus meningkat di dunia dan dapat menimbulkan komplikasi berupa ulkus diabetikum. Ulkus diabetikum ditandai oleh gangguan proses penyembuhan luka yang meningkatkan risiko infeksi dan amputasi. Penelitian ini bertujuan untuk mengetahui efektivitas nanospanlastik krim ekstrak etanol 70% kulit batang Parameria laevigata terhadap penyembuhan ulkus diabetikum pada tikus diabetes melitus. Sebanyak 25 ekor tikus putih jantan (Rattus norvegicus) dibagi menjadi lima kelompok, yaitu kontrol negatif, kontrol positif, dan kelompok perlakuan dosis 8%, 16%, dan 32%. Pengamatan eritema dilakukan selama 7 hari, sedangkan evaluasi histopatologi pada hari ke-21 meliputi re-epitelisasi, granulasi, dan remodeling jaringan. Data dianalisis menggunakan uji One-Way ANOVA atau Kruskal–Wallis yang dilanjutkan dengan uji Tukey HSD atau Mann–Whitney. Hasil penelitian menunjukkan bahwa kelompok dosis 16% menghasilkan skor eritema hari ke-7 sebesar 0,60 ± 0,54 dibandingkan kontrol negatif sebesar 3,00 ± 0,70. Kelompok tersebut juga menunjukkan skor re-epitelisasi, granulasi, dan remodeling jaringan masing-masing sebesar 1,48 ± 0,22; 1,48 ± 0,26; dan 1,72 ± 0,22. Berdasarkan hasil yang didapatkan pemberian anospanlastik krim ekstrak etanol 70% kulit batang Parameria laevigata dosis 8%, 16%, dan 32% mampu mempercepat penyembuhan eritema serta memperbaiki gambaran histopatologis ulkus diabetikum pada tikus diabetes melitus. Dosis 16% menunjukkan hasil paling optimal dengan perbedaan yang signifikan dibandingkan kontrol negatif (p<0,05).

Diabetes mellitus is a chronic metabolic disease with a continuously increasing global prevalence and may lead to complications such as diabetic ulcers. Diabetic ulcers are characterized by impaired wound healing, which increases the risk of infection and amputation. This study aimed to evaluate the effectiveness of a nanospanlastic cream containing 70% ethanolic extract of Parameria laevigata stem bark on diabetic ulcer healing in diabetic rats. Twenty-five male white rats (Rattus norvegicus) were divided into five groups: negative control, positive control, and treatment groups receiving doses of 8%, 16%, and 32%. Erythema was observed for 7 days, while histopathological evaluation was performed on day 21, including re-epithelialization, granulation, and tissue remodeling. Data were analyzed using One-Way ANOVA or Kruskal-Wallis tests, followed by Tukey HSD or Mann-Whitney post hoc tests. The results showed that the 16% dose group achieved an erythema score of 0.60 ± 0.54 on day 7 compared with 3.00 ± 0.70 in the negative control group. This group also demonstrated re-epithelialization, granulation, and tissue remodeling scores of 1.48 ± 0.22, 1.48 ± 0.26, and 1.72 ± 0.22, respectively. The findings indicate that nanospanlastic cream containing 70% ethanolic extract of Parameria laevigata stem bark at doses of 8%, 16%, and 32% accelerated erythema healing and improved the histopathological features of diabetic ulcers in diabetic rats. The 16% dose exhibited the most optimal effect, showing a significant difference compared with the negative control group (p < 0.05).

يُعد داء السكري مرضًا استقلابيًا مزمنًا يزداد انتشاره عالميًا بصورة مستمرة، وقد يؤدي إلى مضاعفات خطيرة، من أبرزها القرح السكرية. تتميز القرح السكرية باضطراب عملية التئام الجروح، مما يزيد من خطر الإصابة بالعدوى واحتمال بتر الأطراف. هدفت هذه الدراسة إلى تقييم فعالية كريم النانوسبانلاستيك المحتوي على مستخلص الإيثانول بتركيز 70٪ من لحاء ساق نبات Parameria laevigata في تسريع التئام القرح السكرية لدى الجرذان المصابة بالسكري. أُجريت الدراسة باستخدام 25 جرذًا أبيض ذكرًا (Rattus norvegicus)، وجرى تقسيمها إلى خمس مجموعات، هي: مجموعة ضابطة سلبية، ومجموعة ضابطة إيجابية، وثلاث مجموعات معالجة بكريم النانوسبانلاستيك بتركيزات 8٪ و16٪ و32٪. تمت متابعة درجة الحمامى لمدة سبعة أيام، بينما أُجري التقييم النسيجي المرضي في اليوم الحادي والعشرين، وشمل إعادة تكوين الظهارة، وتكوين النسيج الحبيبي، وإعادة تشكيل الأنسجة. حُللت البيانات باستخدام اختبار تحليل التباين الأحادي (One-Way ANOVA) أو اختبار كروسكال–واليس (Kruskal–Wallis)، تلاهما اختبار توكي (Tukey HSD) أو مان–ويتني (Mann–Whitney) للمقارنات البعدية. أظهرت النتائج أن مجموعة التركيز 16٪ حققت أقل درجة للحمامى في اليوم السابع، حيث بلغت 0.60 ± 0.54 مقارنة بـ 3.00 ± 0.70 في المجموعة الضابطة السلبية. كما سجلت هذه المجموعة درجات بلغت 1.48 ± 0.22 لإعادة تكوين الظهارة، و1.48 ± 0.26 لتكوين النسيج الحبيبي، و1.72 ± 0.22 لإعادة تشكيل الأنسجة. وتشير النتائج إلى أن كريم النانوسبانلاستيك المحتوي على مستخلص الإيثانول بتركيز 70٪ من لحاء ساق نبات Parameria laevigata بتركيزات 8٪ و16٪ و32٪ أسهم في تسريع شفاء الحمامى وتحسين الخصائص النسيجية المرضية للقرح السكرية لدى الجرذان المصابة بالسكري، وكان تركيز 16٪ هو الأكثر فاعلية، مع وجود فرق معنوي مقارنة بالمجموعة الضابطة السلبية (p < 0.05).
