Abstract

INDONESIA:

Penyakit jantung koroner (PJK) merupakan penyebab utama morbiditas dan mortalitas di seluruh dunia dan berkaitan erat dengan aterosklerosis, inflamasi, stres oksidatif, serta disfungsi endotel. Bukti terbaru menunjukkan bahwa disbiosis usus berperan penting dalam progresi PJK melalui perubahan produksi metabolit mikroba, seperti trimetilamin-N-oksida (TMAO) dan asam lemak rantai pendek (short-chain fatty acids/SCFAs). Lactobacillus merupakan genus probiotik yang diketahui dapat memperbaiki keseimbangan mikrobiota usus serta memiliki efek antiinflamasi, antioksidan, dan penurun lipid. Penelitian ini bertujuan untuk melakukan tinjauan sistematis terhadap efektivitas suplementasi Lactobacillus sebagai strategi terapeutik pada penyakit jantung koroner. Tinjauan sistematis dilakukan menggunakan artikel yang diperoleh dari basis data PubMed, ScienceDirect, Cochrane, dan Springer berdasarkan kriteria PICO yang telah ditetapkan. Proses seleksi mengikuti pedoman PRISMA, dan kualitas studi yang disertakan dinilai menggunakan alat RoB 2 dan ROBINS-I. Hasil penelitian menunjukkan bahwa suplementasi Lactobacillus berhubungan dengan perbaikan profil lipid, penurunan penanda inflamasi, peningkatan fungsi endotel, modulasi komposisi mikrobiota usus, penurunan produksi TMAO, serta peningkatan kadar SCFA. Sebagai kesimpulan, suplementasi Lactobacillus menunjukkan potensi sebagai strategi tambahan dalam penatalaksanaan penyakit jantung koroner melalui modulasi mikrobiota usus serta perbaikan jalur kardiometabolik dan inflamasi.

Coronary heart disease (CHD) is a major cause of morbidity and mortality worldwide and is closely associated with atherosclerosis, inflammation, oxidative stress, and endothelial dysfunction. Recent evidence suggests that gut dysbiosis plays an important role in the progression of CHD through altered production of microbial metabolites such as trimethylamine-N-oxide (TMAO) and short-chain fatty acids (SCFAs). Lactobacillus is a probiotic genus known to improve gut microbiota balance and exhibit anti-inflammatory, antioxidant, and lipid-lowering effects. This study aimed to systematically review the effectiveness of Lactobacillus supplementation as a therapeutic strategy for coronary heart disease. A systematic review was conducted using articles obtained from PubMed, ScienceDirect, Cochrane, and Springer databases based on predefined PICO criteria. The selection process followed PRISMA guidelines, and the quality of included studies was assessed using RoB 2 and ROBINS-I tools. The results showed that Lactobacillus supplementation was associated with improvements in lipid profiles, reduction of inflammatory markers, enhancement of endothelial function, modulation of gut microbiota composition, decreased TMAO production, and increased SCFA levels. In conclusion, Lactobacillus supplementation demonstrates potential as an adjunctive strategy in the management of coronary heart disease through gut microbiota modulation and improvement of cardiometabolic and inflammatory pathways.
