Abstract
INDONESIA:
Latar Belakang: Sindrom koroner akut (SKA) merupakan salah satu penyebab utama kematian di seluruh dunia. Eksosomal microRNA (exo-miR) telah muncul sebagai kandidat biomarker menjanjikan dalam penyakit kardiovaskular karena stabilitas dan perannya dalam regulasi ekspresi gen. Oleh karena itu, penelitian ini bertujuan untuk mengevaluasi akurasi diagnostik exo-miR untuk diagnosis SKA melalui tinjauan sistematis dan meta-analisis. Metode: Pencarian literatur dilakukan pada basis data PubMed, ScienceDirect, Wiley, dan Google Scholar hingga Februari 2025. Data yang dikumpulkan meliputi sensitivitas gabungan, spesifisitas gabungan, positive likelihood ratio (PLR), negative likelihood ratio (NLR), diagnostic odds ratio (DOR), dan area under the curve (AUC) untuk menilai kinerja tes secara keseluruhan dari exo-miR. Sintesis data dilakukan dengan perangkat lunak Meta-DiSC 1.4, dan kualitas studi dievaluasi menggunakan Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2). Hasil: Sebanyak enam artikel yang melibatkan 483 pasien SKA dan 237 kontrol, yang melibatkan 54 exo-miR dimasukkan dalam tinjauan sistematis dan meta-analisis ini. Hasil meta-analisis gabungan menunjukkan: sensitivitas 0,82 (95% CI: 0,80– 0,85), spesifisitas 0,76 (95% CI: 0,72–0,80), PLR 3,26 (95% CI: 2,54–4,19), NLR 0,21 (95% CI: 0,14–0,31), dan DOR 17,19 (95% CI: 9,98–29,58). Nilai AUC pada kurva summary receiver operating characteristic (SROC) adalah 0,8583, menunjukkan tingkat akurasi diagnostik yang baik. Kesimpulan: Hasil penelitian ini menunjukkan bahwa exo-miR merupakan biomarker potensial untuk diagnosis SKA. Studi lanjutan dengan ukuran sampel yang lebih besar dan desain multisenter diperlukan untuk memvalidasi kegunaan klinisnya sebelum aplikasikan secara luas.

Background: Acute coronary syndrome (ACS) is a leading causes of mortality worldwide. Exosomal microRNAs (exo-miRs) have emerged as promising biomarker candidates in cardiovascular diseases due to their stability and regulatory roles in gene expression. Therefore, this study aimed to evaluate the diagnostic accuracy of exo-miRs for ACS detection through a systematic review and meta-analysis. Methods: A comprehensive literature search was conducted using the PubMed, ScienceDirect, Wiley, and Google Scholar databases up to February 2025. The pooled sensitivity, specificity, positive likelihood ratio (PLR), negative likelihood ratio (NLR), diagnostic odds ratio (DOR), and area under the curve (AUC) were collected to assess the overall diagnostic performance of exo-miRs. Data synthesis was performed using Meta-DiSC version 1.4 software, and the methodological quality of the included studies was assessed using the Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2) tool. Results: A total of six studies involving 483 ACS patients and 237 controls, with a total of 54 reported exo-miRs, were included in this systematic review and meta-analysis. The pooled meta-analysis results were as follows: sensitivity 0.82 (95% CI: 0.80–0.85), specificity 0.76 (95% CI: 0.72–0.80), PLR 3.26 (95% CI: 2.54–4.19), NLR 0.21 (95% CI: 0.14–0.31), and DOR 17.19 (95% CI: 9.98–29.58). The AUC of the summary receiver operating characteristic (SROC) was 0.8583, indicating good diagnostic accuracy. Conclusion: The findings of this study suggest that circulating exo-miRs show promising potential as diagnostic biomarkers for ACS. Further studies with larger sample sizes and multicenter designs are needed to validate their clinical utility before implementation.
