Abstract
ABSTRAK:
Diabetes melitus tipe 2 (DMT2) adalah suatu penyakit metabolik kronis akibat kelainan fungsi sel β pankreas atau adanya gangguan sekresi insulin sehingga menyebabkan resistensi insulin. DMT2 membutuhkan terapi pengobatan yang dilakukan seumur hidup sehingga membutuhkan biaya medis yang besar. Oleh karena itu penting dilakukan analisis farmakoekonomi menggunakan pendekatan Cost Effective Analysis (CEA). Penelitian ini bertujuan untuk mengetahui dan menganalisis efektivitas biaya penggunaan terapi kombinasi antidiabetik Metformin+Long acting insulin dan Metformin+Rapid acting insulin pada pasien DMT2 di instalasi rawat jalan RSUD Karsa Husada Batu. Penelitian ini termasuk penelitian deskriptif observasional dengan data retrospektif berupa data rekam medis dan data biaya pasien rawat jalan DMT2 RSUD Karsa Husada Batu periode Januari-Desember 2023. Analisis data dilakukan menggunakan rumus ACER dan ICER. Hasil dari penelitian ini didapatkan 31 pasien DMT2 memenuhi kriteria inklusi. Parameter yang digunakan yaitu kadar gula darah sewaktu (GDS) dan biaya medis langsung meliputi biaya administrasi, biaya jasa dokter, biaya obat, dan biaya laboratorium. Efektivitas terapi dilihat dari jumlah kadar GDS yang mencapai target dibagi jumlah pemeriksaan kadar GDS dikali 100%. Efektivitas terapi tertinggi ada pada kombinasi Metformin+Long acting insulin (55%). Dan biaya medis langsung tertinggi sebesar Rp. 550.968 ada pada terapi Metformin+Rapid acting insulin. Dari perhitungan ACER didapatkan terapi yang paling cost-effective adalah kombinasi Metformin+Long acting insulin dengan hasil nilai ACER terendah sebesar Rp. 9.918 dan nilai ICER sebesar -Rp.222 terhadap kombinasi Metformin+Rapid acting insulin.

Type 2 diabetes mellitus (T2DM) is a chronic metabolic disease caused by abnormal pancreatic β-cell function or impaired insulin secretion, resulting in insulin resistance. T2DM requires lifelong medication therapy that requires
enormous medical costs. Therefore, conducting a pharmacoeconomic analysis using the cost-effective analysis (CEA). This study aims to determine and analyze the cost-effectiveness of combined antidiabetic therapy of Metformin+Long acting insulin and Metformin+Rapid acting insulin in T2DM patients at the outpatient installation of RSUD Karsa Husada Batu. This study included descriptive observational research with retrospective data in the form of medical record data. It cost data for outpatients with T2DM at RSUD Karsa Husada Batu for the period January-December 2023. Data analysis was carried out using the
ACER and ICER formulas. The results obtained 31 T2DM patients meeting the inclusion criteria. The parameters used were random blood glucose levels (RBG) and direct medical costs, including administrative, doctor service, drug, and
laboratory costs. The effectiveness of therapy was seen from the number of RBG levels that reach the target divided by RBG level tests multiplied by 100%. The highest therapeutic effectiveness was in the combination of Metformin+Long
acting insulin (55%). Furthermore, Metformin+Rapid acting insulin therapy had the highest direct medical cost of Rp. 550.968. The ACER calculation found that the most cost-effective therapy was the combination of Metformin+Long acting insulin with the lowest ACER value of Rp. 9.918 and ICER value of –Rp. 222 against the combination of Metformin+Rapid acting insulin.
