Abstract
INDONESIA:
Ulkus diabetikum merupakan komplikasi kronik dari penyakit diabetes mellitus. Ulkus diabetikum terjadi karena gangguan neuropati yang menyebabkan penyempitan dan penyumbatan pembuluh darah perifer tungkai bawah, khususnya kaki akibat kadar gula yang tinggi. Kondisi ini dapat mengakibatkan sel kekurangan oksigen dan mengalami kerusakan hingga kematian jaringan. Hal ini menyebabkan infeksi sehingga bakteri mudah masuk ke dalam luka. Salah satu pengobatan yang dilakukan menggunakan terapi antibiotik untuk mencegah infeksi. Tujuan penelitian ini yaitu mengetahui profil dan mengevaluasi penggunaan antibiotik pada penderita ulkus diabetikum di RSI Siti Hajar Sidoarjo periode 2016. Penelitian dilakukan secara retrospektif dengan studi deskriptif non eksperimental. Sampel yang diperoleh berjumlah 54 data rekam medis yang memenuhi kriteria inklusi dan eksklusi.
Hasil penelitian menunjukkan penderita ulkus diabetikum paling banyak pada usia 55-64 (54%), jenis kelamin perempuan (61%), dan kategori sedang (46.3%). Pola penggunaan antibiotik meliputi meliputi antibiotik tunggal dan kombinasi. Antibiotik tunggal paling banyak yaitu ceftriaxone (62.5%) dan cefuroxime (17.5%). Sedangkan antibiotik kombinasi yang paling banyak digunakan meliputi ceftriaxone+metronidazole (50%), cefuroxime+metronidazole (14.7%), dan meropenem+ metronidazole (8.8%). Hasil evaluasi ketetapan penggunaan antibiotik yaitu tepat indikasi (100%), tepat obat (77.8%), tepat dosis (77.8%), tepat cara pemberian (100%), tepat interval waktu (74.1%), dan tepat lama pemberian (96.3%).

Diabetic ulcer is chronic complications of diabetes mellitus. Diabetic ulcer neuropathy due to a disruption that causes narrowing and blockage of peripheral vascular lower limbs, especially legs because of the high sugar content. This condition can lead to blockage of blood vessels so cells deprived of oxygen and damage to tissue death. This causes a bacterial infection that easily fit into the wound. One treatment is carried out using antibiotic therapy to prevent infection. The purpose of this research is to know the profile and evaluate the antibiotics use in patients with diabetic ulcers in RSI Siti Hajar Sidoarjo 2016 Period. The descriptive study retrospective non-experimental approach. The samples obtained are 54 medical records that meet the inclusion and exclusion criteria.
The results showed patients with diabetic ulcers at most at the age of 55-64 (54%), female (61%), and the medium category (46.3%). The pattern of the antibiotics include antibiotics single and combination. Most single antibiotic are ceftriaxone (62.5%) and cefuroxime (17.5%). While antibiotics are the most widely used combination includes ceftriaxone + metronidazole (50%), cefuroxime + metronidazole (14.7%), and meropenem + metronidazole (8.8%). The evaluation results statutes antibiotic use is appropriate indication (100%), the right medication (77.8%), the right dose (77.8%), the exact mode of administration (100%), the exact time interval (74.1%), and the exact duration of administration (96.3% ).
